Communication & SMS Terms
When you submit a verification request you choose a preferred contact method. These terms explain how the church will use it.
What we send
- Confirmation that your verification request was received.
- The result of the review: approved, denied, or a request for more information.
- Your benefit code and its expiration, if approved.
- A sign-in link if you request access to your own record.
We do not send medical information, and you should never reply with medical information.
Consent, frequency, and cost
By providing your email address or mobile number and choosing a preferred contact method, you agree to receive messages about your verification request. Message frequency is limited to the messages listed above. Message and data rates may apply. Consent is not a condition of receiving healthcare.
Opting out
Reply STOP to a text message to stop texts, or contact the church to change your preferred contact method. Opting out may prevent us from delivering your benefit code.
